According to the Article N9 of the Russian Federal Law “On Personal Data”, acting of my own free will and in my own interest I give to the the Institute for Theoretical and Mathematical Physics as to the organizer of the Event my consent to process personal data contained in my application, using the automated systems and non-automated processing, as well as to submit my personal data to third parties as stipulated by applicable legislation of the Russian Federation, with the purpose of informational and financial support, including creating a database of applicants and participants.
In accordance to this consent I provide for procession the following personal data: surname, first name, patronymic name, e-mail, contact phone number, education, employment record, place of work (study) and position, scientific degree.
This consent is given to perform any actions on my personal data needed to achieve the abovementioned purposes, including but not limited to collection, recording, systematization, accumulation, storage, clarification (update, modification), extraction, use, transfer (distribution, granting access), depersonalization, blocking, deleting and destruction.
I give my consent to use the following information as generally accessible personal data: surname, first name, patronymic name, date of birth, place of work and position, information on my participation in the Event.
I give my consent to publication of this generally accessible information on the Internet and by other means for purposes stated in this document.
